Welcome to APC’s Employee Wellness Rewards Program!

Our employees are very important to us, and we want to help you improve or maintain your overall wellness! Here you will find everything you need to get started with earning rewards while improving your health and well-being.

  • STEP ONE: Download your very own Wellness Passport (be sure to read the instructions).
  • STEP TWO: Participate in Wellness Activities, such as those listed below.
    • View our Wellness Calendar for upcoming events!
      • Participation in any of these events counts toward your wellness rewards! If you attend, be sure to record it on your passport.
    • Get rewarded for your workouts!
      • Purchase a fitness device to track your activities, or work out at a fitness center
      • Need a place to exercise? APC employees and their households get discounted memberships at all YMCA branches! Click here for details on enrolling at the Y
      • Attend FREE Yoga classes at APC! (employees only)- class times are posted on the  Wellness Calendar
    • Get rewarded for prevention and education activities, too!
      • Participate in a wellness-related training or educational event at APC, in the community, or online. Don’t forget to provide proof when possible. (See the APC Wellness Calendar)
      • Engage in prevention activities, such as going to your doctor for a physical or getting a dental exam (please provide proof with your passport).
  • STEP THREE: Submit your completed Wellness Passport at the end of each quarter to earn your rewards.
    • Follow the instructions on your passport to see where to submit yours.
    • Don’t forget to attach proof of each activity (when possible) per instructions on your passport! Wellness Attestation Form to write
  • STEP FOUR: Claim your gift card!
    • Following the end of each quarter, watch your email for details on how to claim your reward!
